What to Bring to the EDD Office
Required Items
Identification
Must Have:
- Valid government-issued photo ID (CA driver's license, CA state ID, US passport, or other government-issued ID)
- Social Security Number — card not required, but know your full 9-digit number; some offices ask to see the card
Claim Information
- Claim confirmation number
- Disability start date
- Last day worked date
- Any correspondence from EDD
Medical Information
- Treating physician's full name
- Doctor's phone number and address
- Doctor's medical license number (verify at mbc.ca.gov)
Recommended Items
Financial Documents
- Last 3-4 pay stubs
- Most recent W-2
- Bank account information (if setting up direct deposit)
Pilot-Specific Documents
EDD staff may not understand airline pilot compensation. Bringing a written explanation can save time and prevent your claim from being held for clarification.
- Printed pilot pay explanation (template below) — customized with your details
- Sample pay stub with flight pay, per diem, and override pay components highlighted
- Monthly earnings summary for the last 3-4 months
Supporting Documents
- Any letters from EDD
- Previous denial notices (if appealing)
- Doctor's written statement (if available)

Checklist by Purpose
If Filing a New Claim:
- Photo ID

If Following Up on Existing Claim:

Document Preparation
- Bring originals AND copies — never surrender your only copy of anything
- Ask for a dated receipt when submitting documents
- On pay stubs, highlight gross earnings and mark pay period dates so EDD staff can locate the figures quickly
What NOT to Bring

- ❌ Every document you've ever received
